The National Committee for Quality Assurance (NCQA) defines the Patient Centered Medical Home as “a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner”.

  • Effective Care Management: Through the use of evidence based guidelines, we will monitor care plans and goals while providing patients with appropriate tools, health education and resources to effectively manage their own health. Care and quality improvement will also include use of technological advances, clinical outcome analysis, risk and medication management.
